2 pictures linked below. The server normally has a pci-e riser card fitted, but we have been testing with the 6205 card plugged directly into the blue pci-e slot on the motherboard.

Hi Hovis

After our test AS-Rock IMB-180 (single x16 PCI-E slot) with 6205 and 6905 every time we reboot all is ok. so strange that why you have problem.
We test in widows too. which is ok.
as you can see from the picture.

Can you have a try test your 6205 in another mainboard check whether is ok. and we will have more check with this too.

Hello, we have tried the card on another identical server (and identical motherboard) and have the same issue.

Our BIOS version is the same as yours - 2.15.1234 and UEFI version "IMS-180 P1.30" (see *1 photo below). We have also done a BIOS reset ("Load UEFI Defaults") (see *3 photo) and then disabled the "Rapid Start Technology". Our motherboard is connected to the PSU via the ATX12V connector rather than the DC connector at the back (you can see this on our previous photos).

Another difference is your BIOS screen shows that your CPU does not have support for "VT-d" where as ours does (but it is disabled in the bios) - see *2 photo below.

When you issue a reboot (ie 'sudo reboot') from Ubuntu (12.04.4 64bit), does your CPU fan stop for half a second during the reboot? We have found that if we make certain changes in the BIOS (ie PCI Gen Speed), the motherboard will then do a full reset (causing the CPU fan to briefly stop) and then the TBS card is then present. If we do a reboot in Ubuntu, the CPU fan never stops during the reboot and the TBS card is no longer present?

Hi

We tried with sudo reboot command with ubuntu 14.04 x86 system,during the reboot the fan never power off always on
and after boot ,all is ok too,we can see TBS6205.

later we will have a try with ubuntu 12.04.4 64 bit as you .

Turned out in the end to be the card. A replacement card worked perfectly across reboots.
