This problem had been solved,Moving the 6981 card into the x16 pcie slot solved it.

Maybe it relate with the PCIE slot,Please try the PCIE x16 slot,or change to other slot.
Not all the slots are the same.

I can't find any message relate with TBS 6921 hardware,it seems your main board can't find the TBS 6921.
Please try to clean up the 6921's PCIE connection golden pin with rubber .
Or you can try to update the main board's BIOS,some earlier BIOS don't support PCIE1.0a.

Hi,
It looks like the card has some hardware problem with it.
We should do the RMA for you.
BTW,the Transedit show the error message for you,it also can't lock the frequency at all,but it still can receive some TS data from the tuner.

Change interrupt mode from IRQ to MSI

Hi,
we found when 4 tuners all in work stage,the MSI interrupt mode could be more stable than legend IRQ mode.
Please try to download the latest windows driver for TBS6984.

Please try to install X64 bit Ubuntu.
Some main board have unexpected issue with x32 bit version.

The original version of windows server 2008 don't support BDA driver.
