Cant see my 3rd 6909 card

Hi, i need some help.
I have 3 6909 installed on a debian 9, they all worked without any problem, but suddenly one of the cards doesn't appear on the list any more.
i have tried reinstalled the drivers (both online driver installation and offline),firmware update, unplugged the cards nothing is working.
i have tested the cards one by one and they seem to work if they are connected one at the time.

PS i have both the version, lspci and dmesg but cant upload it as it says that the *.txt extenstion is not allow: The extension txt is not allowed
