Dead Card???

It's been working fine for months, never skipped a beat. Then, all of a sudden, it stops working. Reboot the server (fedora 15) and the card is absolutely no where to be seen.
I try it on a vanilla Windows pc and it also fails to see the card.
Can this be fixed or has it died for ever?
