OK, I'll switch cards (I have another slot used by a DVB-T card) over the weekend and have a go.

Unfortunately I haven't noticed any triggering event - most of the times when the BSOD happens the HTPC is simply on; sometimes it is recording; sometimes I am watching a video. And no I only have one HTPC, sorry.

Steven, I might have identified the problem - I believe it is the DiSEqC communication between the TBS 8921 card and my DiSEqC switch.
